Support at home extends beyond personal care, and this policy addresses social inclusion and community participation. Taking a strengths-based approach, it covers meaningful activity and occupation, relationships and connection, reducing isolation and loneliness, aspirations around work, education and volunteering, and practical help with access and getting out and about.
