Comprehensive Care Needs Assessment policy

The purpose of assessment and how to carry out a holistic, whole-person assessment are set out in this policy. It looks at strengths, outcomes and what matters to the person, involving them in the process, drawing on assessments carried out by others, and dealing with capacity, communication and accessibility. Moving from assessment to plan is covered, with a template annexed.
