Care and Support Planning policy

Turning an assessment into a working care plan is what this policy addresses, including what a good plan contains and how it is co-produced with the person. Plans need to be clear enough for any worker to follow safely, and the policy covers consent, capacity and agreement, keeping the plan current, and daily notes. A plan template is annexed.
