Incident Reporting and Learning policy

Reporting is only half of it; the learning that follows is the other half, and a just culture underpins both. The policy defines what must be reported and how, the immediate response to an incident, grading and triage, statutory notifications to the CQC and other bodies, and the duty of candour.
